Many years of chasing NOS parts and this is the only NOS Jack I have ever seen. It is truly a "jack in the box" no pun intended. It is serviced as jack and base less handle according to a friends book. The base and mechanism have stickers with 1372498 and 497. The tag on the box is # 1396639 and I assume is the kit or assembly number. The base is near flawless. The post does have a couple spots where it has rubbed the box and sort of messed up the finish a little. It has flash ruse it appears in those spots. The mechanism has some superficial scratching in the black paint but not hateful. I was told that it is probably the only one on the planet but I know little about GM parts. Only trying to rescue many from and old dealers stock. And there are many. If this were Mopar and fit a 68-70 B Body it would be gold. It will be packaged with care and sent via FedEx.
